Alignment is one of the few things on a rotating machine that can be done correctly and still be wrong four hours later.

Nothing has to go wrong for it to move. It only has to get hot.
You set it cold on a quiet shutdown morning. Then you ask it to run hot, on a foundation that moves, with pipework pulling on the casing. A machine can be aligned perfectly on a cold morning and be out by lunchtime.
Thermal growth. Soft foot. Pipe strain. Grout settling over a season. A coupling quietly absorbing the offset until it can't.
Nothing was done wrong. It simply is not where it was left.
A cold alignment record is not a statement about how the machine is running. It is a statement about how it was assembled — and the bearings only ever respond to the first. The bearings only ever feel the hot number.
On your critical pumps, is thermal growth allowed for in the alignment targets, or is the machine set zero-zero cold and left there?

Checking rotating equipment in the state it actually runs in is part of what OMNIX does. If a pump keeps eating bearings or seals while the alignment record says it was set correctly, that gap is worth investigating. If you have a machine that keeps failing on bearings and nobody can say why, that is what a failure investigation is for. The failed assembly is opened and examined where it failed, measured against limits, and the contributing factors — operating, maintenance or installation — are separated, not blended, ending in a prioritised list of what has to change so it does not happen again. On machines under equipment health monitoring, vibration, temperature and performance are trended against a baseline set on the first visit.
Cold alignment is what you set. Running alignment is what the bearings actually experience.
